imbalance in the impeller that went flying, at high rpm is ripped it apart
ding ding ding. close enough = you are teh winner. iou one internetz.

compressor disc (fragments of which are in the last two pics) was dropped on the floor during the assembly process of the engine. this incident wasnt disclosed to the inspection department so they never thought to look for the microscopic cracks that had formed. upon the high power run, the disc burst, sending the fragment through the engine case, through the fuselage, and embedding itself in the second engine. nobody hurt, millions in damages.

 
happened at LAX.

flight crew reported engine was not preforming properly in flight, mtc was performing high power runs to collect data.

for what its worth, the 767 is a fantastic aircraft.

true true, however it is worth noting that all major airliners are capable of losing an engine and landing safely. even the ones that fly overseas.

the fire however, is another issue. we may never know if the engine fire suppression systems would have put this fire out at altidude, but the non disputable point is, nobody was hurt
